Submitted By - Jemmeroni 28/05/2012 20:51:17 As a part-time , I am part of an ambulance crew which takes very chronically-ill people home from a number of hospitals. Some hospitals are very good at advising patient carers about what's available but some are not and you have to be assertive about it, preferably some days in advance if discharge. 

Some hospitals, of course, discharge at very short notice and, as an ambulance crew, we do our best to help a partner, relative or carer access services, public and private. Just think ahead as far as you can.

I know when my elderly Aunt was discharged this was all in place before discharge - carer morning and evening to help dressing making breakfast, shopping etc and meals on wheels at lunch time.  Really took the pressure from the family
